Southern Ave., Tempe Major violations: 7 Inspector’s comments: The cutting board at the food prep table was soiled; cooked pork not date-marked; manager not certified and numerous repeat critical violations noted on this inspection; acidified rice made for sushi/kim bop not documented for discard; establishment is making kim chee for sale in the retail area; condensate (steam that has been condensed back into water) is pooling on the floor of the walk-in cooler; food stored directly on the floor; prepared food not labeled with food name, ingredients, name of establishment, telephone number, and weight or amount; raw bacon found on counter not held at proper temperature; no food service cards available for employees. TEAKWOODS TAVERN & GRILL Address: 5965 W. Ray Road, Chandler Major violations: 8 Inspector’s comments: Cooked corn chowder was cooling for three hours, not cooled to the proper temperature within the required time/temperature range; cooked chicken wings packed at room temperature not held at 41 degrees or below; employees not following proper hand- and arm-washing procedures; staff was drinking cola while packing chicken wings; staff packaged chicken wings with bare hands; soda guns and holders were coated with black residues; cooking equipment greasy and built up with food particles; cooked, potentially hazardous food items found without date-marks; obnoxious odor in kitchen; excessive number of cigarette butts at the front entrance and within 20 feet from the front door. Val Vista Drive, Gilbert Major violations: 5 Inspector’s comments: Flies in kitchen; pizza dough maker, pizza slicer, pizza slider, can opener, knife on magnetic strip and lemonade container not clean to sight and touch; hot dogs and turkey not properly date-marked; cutting boards with stains and deep grooves; reheated assorted food items in steam table. Gilbert Road, Mesa Major violations: 5 Inspector’s comments: Raw fish stored over cooked shrimp and guacamole; unsliced, unwrapped deli ham stored on reach-in shelves; hand-held plunger blender soiled with dried food debris and stored on floor; opened doughnuts stored over tomatoes and radishes in cooler; no covers on foods stored in main reach-in cooler; cooked rice, cooked beans, cooked beef, cooked pork and cooked shrimp not dated; knives at prep table in front of cook line stored between countertops; the wood was soiled with food debris and was not easily cleanable; grocery bags instead of food grade bags used at hot holding table with cooked rice; employee medicine stored above prep line in front of cook line; lighter for grill stored in plastic basin with ready-to-eat tortillas. |
